When we buy airline tickets or take a payment for your package on your behalf, we need your written permission to charge your card. This form gives that permission for one payment only, for the amount and purpose you write on it. You can type your other details on screen or fill everything in by hand. After printing, write your card number, expiry date and security code by hand, and sign in ink: typed signatures aren’t accepted.
